MANUAL DEFIBRILLATOR
INTRODUCTION
A manual defibrillator is a medical device used for the
treatment of cardiac problems. Depending upon the patient’s
condition and medical recommendation, the medical expert
would deliver voltage shocks to the heart by placing the pads
on the chest of the patient. It is used for various cases of
cardiac conditions such as cardiac, ventricular fibrillation and
ventricular tachycardia.
WHAT IS MANUAL DEFIBRILLATOR?
A manual defibrillator is a type of defibrillation
machine that is used to deliver the shock to the patient
who is a victim of sudden cardiac arrest caused by the
heart rhythm disorder caused by ventricular fibrillation
or pulse-less tachycardia.
PARTS OF A MANUAL DEFIBRILLATOR
Monitor:- The monitor will show the current ECG
waveform, heart rate, etc of the patient. Also, it will
show the energy selection, energy delivered,
impedance value, battery status, etc.
Keyboard:- There is a number of keys present in the
machine. Few of them are stated in the following topic.
External paddles:- There will be a single pair of
external paddles. In the paddle, may have the function
of charging and discharging the energy. In other words,
we can use the external paddles for delivering a shock
to the patient, may be able to use the paddles for both
adult and pediatric patients.
ECG:- The ECG leads can be either III lead or V lead types. It measures
the rhythm of the heart. Looking at this waveform, the doctors decide
whether defibrillation is needed or not.
Battery:- It is necessary to operate the machine in the
absence of AC current.
Printer:- May have an inbuilt thermal printer.
FEATURES OF A MANUAL DEFIBRILLATOR
ENERGY SELECT:- This is for the selection of energy for
delivering to the patient.
CHARGE:- The charge function allows to charge the
machine to the set energy level. The set level may
range up to 300 or 360 Joules.
SHOCK:- The shock function allows to deliver the
energy to the patient. For this, we need to, first of all,
charge the machine.
DISARM:- This is similar to the SHOCK function. But, in
this case, the energy is internally discharged to the
machine. This function is useful when we have already
charged the machine but have no intention of
delivering it to the patient. Discharging internally will
reduce the chances of hazards to the patients and the
operators.
SYNC:-The SYNC function helps to synchronize the
machine with the ECG rhythm of the patient. If we
have pressed the SYNC function then the machine may
not deliver the shock even after pressing the shock
button. This is because the machine will synchronize
with the peak of the R-wave. Once the peak of the R-
wave in the patient’s QRS complex is achieved, the
shock will be delivered to the patient.
How to use a manual defibrillator?
1.Turn on the machine.
2.Apply the electrode gel (ECG gel/ USG gel) on the
chest of the patient. Then, with the help of the paddles,
rub the gel around the chest of the patient.
3.Select the energy level that we want to deliver to the
patient.
4.Then, place the paddles on the chest of the patient.
Place the sternum paddle on the patient’s chest inferior
to the patient’s right clavicle and lateral to the upper
sternum. Also place the APEX handle in the anterior
axillary’s line, inferior and lateral to the patient’s left
nipple.
5.Then press the CHARGE button. In the process, the
charging of the machine takes place.
6.Once the charging has been completed, press the
SHOCK button to deliver the shock to the patient.
7.Observe the ECG signal after delivering the shock.
8.We can give an additional shock if needed.
9.Once the operation of the machine is over, turn it off.
10.Clean the machine and store it in a safe place.
PRECAUTIONS
 Do not use a mobile phone near the defibrillator
machine. Else the radiation field produced by the mobile
phone will interfere with the function of the machine.
 Do not expose the machine to the electric and magnetic
fields. For this, keep the machine away from X-ray, C-arm,
MRI, etc.
 Stay away from the chances of getting the shock hazards.
Use the battery of the machine properly
CLEANING
 Make sure to turn off the machine before clean it.
 Clean the machine using 70% isopropyl alcohol, or soap water.
Collins can also be used.
 Do not use solutions such as ketenes or other flammable agents.
 Do not autoclave the accessories and the machine unless it has been
specified.
 Do not immerse any part of the external paddles for the purpose of
cleaning.
