Finished refactoring ModelManager, now there is no circular dependency with JsonApiFormatter, and the interface boundary is much more clear.

Finished removing `GetPropertyMap`, adds `GetProperties` to accomodate one of the use cases. Added tests.

Incorporated feedback from #25, including removing unneeded default constructors. Added caching to `IsSerializedAsMany` and `GetElementType`. Made `ModelManager` a little more subclassing-friendly.

Much more refactoring. Moved PluralizationService, still have to move IsMany and GetSingleType.

Merged upstream

Refactored to make ModelManager not a singleton, rather a instance variable of JsonApiFormatter. Adds an IModelManager interface to allow the user to create their own ModelManager.
