  1. Move files around so that it can be used as the confdir for confd 

    Rewrote example toml and tmpl to allow for selecting multiple applications.
Added note in the readme about how to filter by application or to list
all applications.

When listing all, it does not distinguish between them in the haproxy output.
This will need to be fixed later.
