feat(chat): persist configurable default reasoning effort Add chat.reasoning_default using the shared effort enum, with extra-high as an alias for xhigh. Initialize model-less sessions from the default while preserving explicit session levels and Off, including choices made before sending. Retain reasoning suffixes during provider resolution so session persistence does not silently reset effort. Normalize unsupported-model tracking to base IDs and prevent stale session restoration from overwriting toggle choices.

fix(telegram): expose shared-chat tool policy controls Telegram inherited the gateway's public-audience and deny-all defaults without exposing the account settings that can lift them. Shared chats therefore could not enable MCP tools even with permissive tool policies.\n\nWire the existing untrusted audience and tool policy enums through Telegram configuration, runtime views, and redacted serialization, matching Slack. Preserve restrictive defaults, private-context isolation, and privileged-command authorization. Add focused config and gateway regression tests and document the account-wide opt-in.\n\nFixes #1264
